If you are the partner producing sperm, or you are reading on behalf of one, you have probably wondered whether the weekly sauna, the daily hot bath, or the laptop on your thighs matters. This is the version of that conversation I have in clinic. Heat exposure is one of the few "lifestyle" inputs for male fertility with consistent, reversible evidence behind it. Asking the question is a reasonable thing to do.
The short answer on what is good for sperm health when heat is the variable: spermatogenesis requires testicular temperature a few degrees below core body temperature. Habitual heat exposure (frequent saunas, hot tubs, hot baths, occupational heat) measurably impairs sperm count, motility, and morphology in cohort and human-exposure studies. The effect is reversible in many cases over the roughly twelve weeks it takes to produce a new generation of sperm. Single exposures matter less than habits.
The biology in one paragraph
The testes sit outside the body for a reason. Normal testicular temperature runs roughly two to four degrees Celsius below core body temperature, in the range of 33 to 35 degrees Celsius. The anatomy is designed around maintaining that gradient. The scrotum positions the testes away from the trunk. The pampiniform plexus runs venous blood close to incoming arterial blood as a heat exchanger. The cremasteric reflex pulls the testes closer to the body in cold and lets them hang lower in heat. Spermatogenesis, the production of sperm from spermatogonia through to mature sperm, is exquisitely sensitive to that temperature. Sustained or repeated elevation impairs the process, lowers count, motility, and morphology, and increases sperm DNA fragmentation. The WHO laboratory manual for the examination and processing of human semen, sixth edition, frames this physiology directly.7
The good news embedded in the biology is the time course. A full cycle of spermatogenesis takes about 74 days, plus 10 to 14 days of epididymal transit. If habitual heat exposure stops today, you are looking at meaningful improvement in semen parameters at about 12 weeks, not at week one.
What is good for sperm health on heat: the evidence
Hot baths and saunas
Shefi and colleagues studied men with infertility and habitual wet-heat exposure (hot baths, hot tubs, whirlpools for 30 minutes or more per week). Among men who stopped the exposure, roughly half showed significant improvement in semen parameters.1 The mean total motile sperm count rose substantially. That is not a randomised trial, and the men were a selected population, but it is one of the cleaner reversibility signals in the literature.
Garolla and colleagues did the inverse experiment in healthy volunteers. They had men sit in a sauna for 15 minutes, twice a week, for three months, and tracked their semen parameters. The exposed men showed significant declines in sperm concentration and motility.2 Parameters returned toward baseline once the sauna exposure stopped. The magnitudes were not subtle: by months three to six after cessation, much of the lost count and motility had recovered.
The takeaway in clinic is straightforward. An occasional sauna will not change your fertility. A sauna or hot-tub habit, sustained over months, often does, and the change is reversible if you stop in time for a repeat semen analysis at the 12-week mark.
Hot tubs and jacuzzis
Hot tubs and jacuzzis typically run at 38 to 40 degrees Celsius, well above the temperature at which spermatogenesis is comfortable. The physiology is the same as the sauna data. If the partner producing sperm is in a hot tub multiple times a week, this is the lifestyle input I would drop first in the pre-trying window.
Laptop on lap
Sheynkin and colleagues measured scrotal temperatures in healthy men using laptops on their laps in various positions, including the common leg-rest position. Scrotal temperature rose significantly within an hour, and the rise was sustained the longer the laptop sat there.3 Sustained use over years could plausibly affect semen parameters; the effect size in real-life patterns is modest but consistent. If laptop work is part of your day, a desk, a lap tray, or a laptop stand is a low-cost fix.
Occupational heat exposure
Bakers, welders, foundry workers, and professional drivers (truck, taxi, ride-share) with prolonged seat heat have shown worse semen parameters in cohort studies. The physiology combines ambient heat, scrotal contact heat, and prolonged sitting. The exposure is not always avoidable, but it is worth flagging to your clinician if the semen analysis is borderline and the occupation fits the pattern. Cooling adaptations help: regular standing breaks, breathable seat covers, breathable clothing, and where possible cooler vehicle cabins.
Tight clothing and underwear
Mínguez-Alarcón and colleagues, working with men attending a fertility centre, found that men who wore predominantly boxers had higher sperm concentration and total count than men who wore tighter underwear, with serum FSH levels also differing in a pattern consistent with feedback from impaired spermatogenesis.4 The likely mechanism is scrotal temperature. The effect is modest, and switching is cheap and easy. In clinic I do not make a fuss about this in isolation, but if other heat inputs are in play, switching to looser underwear is one more small lever.
Long-distance driving
Long-haul drivers have higher scrotal temperatures and worse semen parameters in some cohort studies. Stand breaks every two hours, breathable seat covers, and a cooler cabin temperature are the practical adaptations. If your job is long-distance driving and the semen analysis is borderline, occupational heat is part of the picture.
Febrile illness
A fever above 38 degrees Celsius lasting 24 hours or more can suppress spermatogenesis for around 60 days afterwards.5 One bout of flu does not cancel your fertility, but it does delay the window in which the semen analysis will reflect any other lifestyle work you have done. If the partner producing sperm has had a significant febrile illness recently, push the repeat semen analysis out by a couple of months rather than testing in the dip.

What is NOT a deal-breaker
It is worth being explicit about this because the heat-and-sperm conversation can spiral into anxiety.
- An occasional sauna or hot bath. The data are about habits, not single events.
- A pool swim, including warm-weather holidays. Pool temperatures are far below the threshold.
- Normal indoor heating, a brief warm shower, or wearing jeans in everyday life.
- Mobile phone in a trouser pocket. There may be a small thermal effect, and the EMF literature is mixed; in the hierarchy of inputs, this is rounding compared to a weekly sauna or a hot-tub habit.
When patients ask me what to drop and what to leave alone, I tell them to optimise the habits, not the one-offs.
What this means for the 90-day window
The clinical playbook for the 90 days before TTC, if the partner producing sperm has a heat-exposure pattern:
- Stop the recurrent hot-tub or sauna habit at least three months before TTC, before a planned semen analysis, or before an IUI or IVF cycle.
- Move the laptop off the lap. Use a desk, a tray, or a stand for prolonged work.
- Switch from tight underwear to boxers or looser briefs if currently in tight underwear.
- If the occupation involves heat exposure, build cooling breaks into the day where the schedule allows.
- Avoid a hot-bath habit immediately before and during the trying months.
- Skip the daily long hot showers when other inputs are also in play.
That is the list. It is unglamorous, it is free, and the magnitude of the effect in the studies is larger than the magnitude of any sperm supplement on the market.
Recovery time
Once habitual heat exposure stops, semen parameters typically improve over 8 to 12 weeks, in line with the spermatogenesis cycle.7 I usually recommend a repeat semen analysis at 12 to 14 weeks after the change, not earlier; testing too soon will sample sperm produced under the old conditions and will look falsely unchanged. It helps to know how to read a semen analysis before you compare the before and after.
Severe or longstanding exposure may take longer, and a small minority of cases will not fully reverse. That is not a reason to skip the intervention; the magnitude of improvement at 12 weeks is the most reliable readout, and if it is incomplete that is information your urologist or RE can use.
What to do this week
Concrete and short.
- Make a list of the partner producing sperm's weekly heat exposures: sauna sessions, hot tubs, hot baths, laptop-on-lap hours, time in a hot vehicle cabin, occupational heat.
- Cut or substitute the largest items first. A weekly sauna becomes a cooler steam room or a skipped session. Hot baths become warm showers. A laptop-on-lap habit becomes a laptop on a desk.
- Switch underwear if currently in tight briefs.
- Plan a repeat semen analysis at 12 to 14 weeks if the current one is borderline or if you have not had one and are entering the pre-trying window.
When to involve your urologist or RE
- Semen analysis abnormalities persisting after 12 to 14 weeks of consistent change.
- A symptomatic varicocele: heaviness in the scrotum, a visible vein, or asymmetric testicular size. Varicocele involves a related but distinct heat physiology and may need urological evaluation in its own right.
- Recurrent fevers or occupational heat exposure that cannot be modified, where biomonitoring or workplace adjustments may be relevant.
- Any abnormal semen analysis in the context of risk factors (chemotherapy history, undescended testis, prior surgery, recurrent epididymitis). Heat is one input; it is not the whole conversation.
