If you have a polycystic ovary syndrome (PCOS) diagnosis or strong PCOS-pattern labs, and your cycles run thirty-five to ninety days long, you have almost certainly read about inositol. The honest answer: myo-inositol at 4 grams per day in a 40:1 ratio with D-chiro inositol is the one over-the-counter supplement with consistent evidence for improving ovulation in PCOS. The 2023 International Evidence-based Guideline acknowledges it cautiously. It is not a cure. It is one of the few inositol and myo inositol regimens with the trial data to earn a place in the conversation.

I want to walk you through what the evidence actually says, where it falls short, how inositol fits with metformin and letrozole, and the specific dose and ratio that the trials used. There is also a confusing product on the market (pure high-dose D-chiro inositol marketed as "advanced") that the data suggest is the wrong choice. I will address that directly.

Why inositol and myo inositol is in PCOS conversations

PCOS involves insulin signalling dysfunction in most phenotypes, including normal-weight or so-called lean PCOS. The metabolic dysregulation is what links the elevated androgens, the anovulation, and the long-term cardiometabolic risk into a single syndrome.

Myo-inositol is one of nine inositol isomers and functions as a second messenger in insulin signalling. It is found in food (citrus, legumes, whole grains, organ meats), is also synthesised endogenously, and is concentrated in tissues that are heavily insulin-responsive, including the ovary. D-chiro inositol (DCI) is a different isomer that the body produces from myo-inositol through an epimerase enzyme, and it plays its own role in glucose handling.

Mechanistically, supplementing myo-inositol can improve insulin sensitivity, lower circulating androgens, and restore ovulation in a meaningful fraction of people with PCOS. This is not a hand-waving "supports balance" claim. The pathway is mapped. The drug-target relationship is real.

This is also why inositol sits in a different evidence tier from most supplements people are sold for PCOS. It has multiple randomised controlled trials, two Cochrane reviews, several meta-analyses, and acknowledgement in the international PCOS guideline. That puts it in different company from royal jelly or "fertility blends."

What the evidence says, honestly

I want to lay out the evidence base in the order that matters clinically.

  • Cochrane review: Showell and colleagues' 2018 Cochrane systematic review on inositol for subfertile women with polycystic ovary syndrome found that myo-inositol may improve clinical pregnancy rate compared with placebo, with low-to-moderate certainty evidence and heterogeneous trials.2 The follow-up 2020 antioxidants review included similar findings.
  • Meta-analyses: Unfer and colleagues (2017) pooled randomised trials of myo-inositol in PCOS and found consistent improvements in ovulation rate, menstrual regularity, fasting insulin, the homeostatic model assessment of insulin resistance (HOMA-IR), and the free androgen index.3 Zeng and Yang's 2018 meta-analysis reached similar conclusions on metabolic and reproductive parameters.4
  • Guideline position: The 2023 International Evidence-based Guideline for the Assessment and Management of PCOS, led by Teede and colleagues, acknowledges inositol as an experimental therapy with a favourable side-effect profile. The guideline notes that women may choose to use it given low risk of harm, but does not strongly recommend it as a first-line treatment over letrozole or metformin for established indications.1 That is a careful position, and I think it is the right one.
  • Live-birth data: Honest disclosure, live-birth data for myo-inositol in spontaneous conception (rather than in IVF or alongside ovulation induction) are limited. The effect size on spontaneous live birth in unselected PCOS is modest, even where the metabolic and ovulation signals are clear.
  • Combined with metformin: Facchinetti and colleagues (2019) pooled short-term trials of metformin versus myo-inositol or both, and found similar metabolic improvements with myo-inositol and metformin, with much better tolerability for inositol.7

The combination of an actual mechanism, multiple RCTs, two Cochrane reviews, and a cautious guideline endorsement is more than almost any other over-the-counter fertility supplement can claim. That said, this is not a guarantee, and the effect size is modest. Setting expectations matters.

Myo versus D-chiro: the ratio question

This is the most muddled part of the inositol marketplace, so I want to spend a moment on it.

Healthy ovarian follicular fluid contains myo-inositol and D-chiro inositol in roughly a 40:1 ratio, with myo-inositol by far the dominant form. PCOS ovaries appear to have an altered ratio: relatively more D-chiro and less myo, possibly because of an over-active epimerase driven by hyperinsulinaemia. The reproductive trial data that have shown the most consistent benefit have used a 40:1 myo-to-DCI formulation. In practice that means 2,000 milligrams of myo-inositol plus 50 milligrams of D-chiro inositol, taken twice daily. Daily total: 4 grams of myo plus 100 milligrams of DCI.

The counterintuitive part is that pure D-chiro inositol at high doses (500 to 1,000 milligrams per day) has been associated with paradoxically poorer oocyte quality in some IVF cohorts. Carlomagno, Unfer, and Roseff named this the "D-chiro inositol paradox of the ovary" in a Fertility and Sterility commentary in 2011.6 Pure D-chiro is not the "advanced" or "upgraded" version of myo, despite some product marketing. The 40:1 myo-dominant formulation is the right default, and high-dose pure D-chiro is the form to avoid for PCOS fertility.

When you read a product label, look for:

  • Myo-inositol around 4,000 milligrams daily total dose.
  • D-chiro inositol around 100 milligrams daily, in a roughly 40:1 ratio.
  • Avoid products that are primarily D-chiro at 500 milligrams or more, marketed as "advanced inositol" or similar.

Who is most likely to benefit

Inositol is not equally useful in every PCOS phenotype. The cohorts where the trial benefit is most consistent:

  • PCOS with insulin resistance markers: Raised HOMA-IR, raised fasting insulin, acanthosis nigricans on the neck or armpits, family history of type 2 diabetes. These are the patients in whom the metabolic improvements are largest.
  • Anovulatory or oligo-ovulatory cycles: Patients with cycles longer than thirty-five days, irregular cycles, or absent cycles. Restoration of ovulation is the most concrete reproductive outcome reported.
  • Any BMI: Inositol benefits have been observed in both lean and higher-BMI PCOS phenotypes. It is not a "weight-loss-first" supplement.
  • Patients who cannot tolerate metformin: The most common reason patients come off metformin is gastrointestinal (GI) side effects: nausea, loose stools, abdominal cramps. Inositol is much better tolerated and is a reasonable alternative or adjunct to discuss with your clinician.

How inositol fits with metformin and letrozole

This is the question I get most often in clinic.

Inositol and metformin both target insulin sensitivity, but through different mechanisms. They can be used together (and several trials combine them), and they can be used separately. If you are already on metformin and tolerating it, adding inositol is reasonable but not essential. If you are on metformin and struggling with GI side effects, switching to inositol-only or to a combination is a sensible conversation with your clinician.

Inositol and letrozole: Inositol does not replace letrozole or clomiphene for inducing ovulation if your cycles are anovulatory and you are actively trying. Letrozole is the first-line pharmacological ovulation induction agent for PCOS, supported by the same 2023 international guideline.1 Inositol can be a baseline metabolic and reproductive support while letrozole does the cycle-by-cycle work of triggering ovulation.

Timing for medicated cycles: If you are starting medicated cycles in sixty to ninety days, beginning inositol now is a reasonable move. The metabolic effect of inositol builds over eight to twelve weeks, and starting earlier gives you a fuller benefit by the time the cycle begins.

What I will often do in practice: when a PCOS diagnosis is newly confirmed and there is no contraindication, I start inositol at the same visit while we discuss whether metformin or letrozole are also indicated, and whether the patient is ready to begin medicated cycles. Inositol is a low-side-effect baseline. Letrozole is the cycle-specific tool.

Dose, form, and side effects

The dose that the trials have consistently used:

  • 2,000 milligrams of myo-inositol twice daily, total 4 grams per day.
  • 50 milligrams of D-chiro inositol twice daily, total 100 milligrams per day, in the 40:1 ratio.
  • Taken with food, in the morning and the evening.

Both powder and capsule formulations are fine. Powder dissolves in water or juice, is usually cheaper per dose, and may be easier for people who cannot swallow large capsules. Capsules are more portable and easier to remember. Pick whichever you will actually take twice a day for at least twelve weeks.

A practical tip: start at half dose for three to five days to assess GI tolerance, then titrate up to the full 4 grams. The most common side effects are mild bloating, gas, and occasional loose stools at higher doses, and they usually settle within the first week. Inositol is far better tolerated than metformin, which is one of its real advantages for daily use.

Time to noticeable cycle change is typically eight to twelve weeks. Do not assess effect at week two and conclude it is not working. The metabolic system needs time. I tell patients to track cycle length and ovulation signs (basal body temperature charting or luteinising hormone testing) over a three-month window before judging.

What inositol does NOT do

There are four things I want to be explicit about, because the marketing routinely overpromises.

Inositol does not cure PCOS. PCOS is a long-term metabolic, reproductive, and dermatological syndrome. Inositol is one tool that supports one part of the picture (insulin signalling). It is not a definitive treatment for the syndrome itself.

Inositol does not replace lifestyle intervention. A Mediterranean-pattern, glycaemic-aware diet combined with regular movement does work that supplements do not. Inositol on top of those lifestyle foundations is more effective than either alone, and inositol without them is doing less than it could be doing.

Inositol does not fix anovulation in everyone. Some PCOS phenotypes (particularly non-insulin-resistant lean PCOS driven by different mechanisms) respond less. If you have been on a complete dose for twelve weeks and your cycles are unchanged, the next conversation is about letrozole or clomiphene rather than a higher inositol dose.

Inositol does not improve fertility in non-PCOS infertility. Trials in unexplained infertility and in diminished ovarian reserve have not consistently shown benefit. The supplement is PCOS-specific in terms of where the data sit. If you do not have PCOS, the case for inositol is weak.

What to do tonight

The practical steps:

  1. If you have PCOS and you are not already on inositol or metformin, start at 2 grams of myo-inositol with food, with a 50 milligram DCI component, and take that dose twice daily. Start at half dose for three to five days, then titrate up.
  2. Anchor the dose to two existing meals (breakfast and dinner) so the adherence sticks.
  3. Track cycle length and an ovulation indicator (basal body temperature, ovulation predictor kits, or cervical mucus changes) over the next eight to twelve weeks. Do not assess effect before week eight.
  4. If you are actively trying and your cycles remain anovulatory beyond twelve weeks, escalate the conversation. Inositol alone is not the right tool for sustained anovulation when you are trying to conceive.

The other useful exercise tonight is to check whether the product you are buying is the 40:1 ratio with myo-inositol as the dominant form. If you see "500 milligrams D-chiro inositol" on the label as the headline dose, that is the wrong product. Switch.

When to talk to your clinician

A handful of situations need clinician input rather than a supplement protocol:

  • Active anovulation while you are trying to conceive: You may need letrozole or clomiphene now. Inositol is not a substitute for ovulation induction in this scenario.
  • Already on metformin and considering adding or switching: Discuss the rationale and the practical dosing schedule with your team.
  • Diagnosis of type 2 diabetes or pre-diabetes alongside PCOS: Inositol is a complement here, not a replacement for first-line diabetes care.
  • Persistent absence of menses despite twelve weeks of full-dose inositol: Time to escalate the workup, not to push the dose higher.
  • Pregnancy: Once you have a positive test, the role of inositol is less clear. Some clinicians continue it through the first trimester for ongoing insulin support; others stop it. Discuss with your antenatal team.

Used as designed, inositol and myo inositol at the 40:1 ratio earns a place in PCOS preconception care because the trial data hold up, the side-effect profile is gentle, and the dose is easy to take consistently. Pair it with the diet and exercise work above for the realistic best case.
