You have a PCOS diagnosis, your cycles are long or irregular, and someone has told you to "just lose weight" without giving you anything practical to work with. You have looked at keto, low-carb, paleo, anti-inflammatory, Whole30, gluten-free, dairy-free, and a stack of PCOS diet supplements and protocols. This post tells you what the 2023 international PCOS guideline actually says about diet, what the trial data show, and what to put on your plate tomorrow that is not punishment.

The honest headline first. The 2023 International Evidence-Based Guideline for the Assessment and Management of Polycystic Ovary Syndrome does not name a single "PCOS diet."1 No single dietary pattern is superior to another for reproductive or metabolic outcomes in PCOS once adjusted for body-weight change. What it endorses is sustainable, individualised eating aligned with general healthy-eating principles, with attention to insulin resistance and a modest weight reduction where BMI is elevated. Anyone selling you "the PCOS diet" with a branded protocol and a supplement stack is selling you something beyond the guideline.

What the 2023 guideline says on PCOS diet supplements

The 2023 guideline is a global effort, co-led by Monash University and endorsed by ESHRE, ASRM, and most national fertility societies. On diet, it makes three points that are worth holding onto.

First, no specific macronutrient split outperforms another over the medium and long term for PCOS reproductive or metabolic outcomes.1 That includes ketogenic, low-carb, low-fat, Mediterranean, vegetarian, and standard balanced diets. The differences between them at six and twelve months are smaller than the marketing suggests.

Second, where BMI is elevated, a modest weight reduction of 5 to 10 percent over six to twelve months is associated with improved ovulation, menstrual regularity, and pregnancy rates. Smaller percentages can be useful too. The guideline does not set a target BMI; it sets a target trajectory.

Third, the diet that you can sustain matters more than the diet that is theoretically optimal. The guideline explicitly endorses individualised approaches and warns against restrictive protocols that increase eating-disorder risk, which is already elevated in PCOS.

That is the whole official position. The rest of what you read online sits outside the guideline.

Why insulin signalling is the lever

Most PCOS phenotypes feature hyperinsulinaemia and insulin resistance, including in lean PCOS where BMI is normal but post-meal insulin is still elevated. Hyperinsulinaemia is the metabolic engine that drives the syndrome:

  • It increases ovarian androgen production.
  • It suppresses sex hormone binding globulin, which raises free testosterone.
  • It contributes directly to anovulation.

The dietary patterns that lower fasting insulin and improve postprandial glycaemic response tend to improve ovulation, menstrual regularity, and HOMA-IR. The levers, in rough order of practical impact, are carbohydrate quality, protein adequacy, fibre, and meal timing. Total calorie restriction matters for weight change, but the metabolic improvements often appear before the scale moves much. The eating patterns with the most data

Mediterranean pattern

The Mediterranean dietary pattern has the strongest overall fertility evidence.4 5 The data come from the Nurses' Health Study II, the SUN cohort in Spain, and the Karayiannis and Vujkovic IVF cohorts. For PCOS specifically, observational data and small randomised trials suggest improvements in HOMA-IR, ovulation, and inflammatory markers when people with PCOS shift toward Mediterranean-pattern eating.

Practically: olive oil as the main fat, fish two to three times a week, vegetables at two or more meals a day, legumes several times a week, intact whole grains, nuts, limited red meat. I cover the pattern in detail in Mediterranean diet and TTC.

When a patient asks me which one diet to try first, this is the answer. It is the most-studied, it is sustainable, and it is compatible with normal social eating.

Lower glycemic load or glycemic index

Marsh and colleagues in 2010 ran a randomised trial in 96 overweight or obese women with PCOS, comparing a low-glycemic-index diet with a conventional healthy diet matched for macronutrients and energy. The low-GI arm showed improved insulin sensitivity and a higher rate of menstrual regularity.3 The benefit was independent of weight loss. Mehrabani and colleagues in 2012 ran a similar protein-modified, low-glycemic-load trial and found favourable changes in insulin markers and androgens.

Practically, lower glycemic load looks like swapping white rice for lentils, choosing intact grains over refined, putting fruit ahead of fruit juice, and reducing sugar-sweetened drinks. It does not mean no carbs. It means better carbs.

Higher protein, moderate carb

Some trials show favourable body-composition and insulin outcomes with around 25 to 30 percent of energy from protein. The evidence is not PCOS-specific, but protein adequacy of roughly 1.2 to 1.6 g per kg body weight supports satiety and lean mass during any weight change. This is a useful lever to layer on top of Mediterranean or lower-GI eating, not a separate diet.

Mediterranean plus low GI, combined

This is what most metabolic-clinic dietitians end up prescribing. There is no magic in the combination. It is the two effective patterns layered: Mediterranean food choices, with intact grains and legumes preferred over refined carbs. Moran and colleagues' systematic review that informed earlier PCOS guidelines pointed in this direction.2

What the evidence does not support as PCOS-specific

Ketogenic and very low carb

Short-term trials show rapid improvements in insulin and weight on ketogenic eating. Adherence beyond six months is poor in most studies. Long-term safety during TTC is unstudied. There is no documented advantage over moderate-carb Mediterranean at 12 to 24 months once weight change is matched.

I do not tell my patients with PCOS that keto is wrong. I tell them it is acceptable if it is genuinely what they can sustain. I have seen plenty of patients try strict keto in cycle one, binge on refined carbs through cycle two, and arrive at cycle three feeling worse than if they had stayed on a moderate pattern the whole time. The diet that lasts beats the diet that is theoretically tighter for four weeks.

Gluten-free and dairy-free without diagnosed intolerance

There is no randomised trial evidence for PCOS-specific benefit from eliminating gluten or dairy when celiac disease and true intolerance have been ruled out. The advice is common on PCOS social media and uncommon in the clinical guidelines for a reason. Restriction without indication tends to reduce dietary quality and increase the cognitive load of eating without a measurable benefit.

If you have undiagnosed celiac disease, particularly with low ferritin, GI symptoms, or recurrent miscarriage, get tested first with anti-tTG IgA. Do not eliminate gluten before testing or the test result is unreliable. The same caution applies to the broader foods to avoid while TTC.

Anti-inflammatory protocols, AIP, Whole30, strict elimination

No randomised trial data in PCOS. Real risk of disordered eating in a population already prone to it.1 The PCOS inflammation story is real at the biochemical level; the branded protocols are not necessary to address it. Mediterranean-pattern eating lowers CRP and inflammatory markers in PCOS without the elimination overhead.

"PCOS detox," candida cleanse, seed cycling, and similar

No evidence in PCOS or general fertility. Skip them. The money is better spent on a registered dietitian session.

Calorie restriction without nutrient adequacy

Aggressive caloric restriction below 1200 kcal per day for extended periods can suppress thyroid function, raise cortisol, and disrupt cycles further. Crash dieting before or during TTC is counterproductive in PCOS, even where weight loss is part of the clinical plan.

The standard target where BMI is elevated is a 5 to 10 percent body-weight reduction over six to twelve months. Smaller percentages can restore ovulation; you do not need to hit a textbook BMI to see improvements. The pace matters as much as the destination.

How to actually build a PCOS-friendly plate

A simple template that works across most eating patterns:

  • Half the plate: non-starchy vegetables. Peppers, leafy greens, broccoli, courgette, salad, frozen vegetables. The least controversial half of the plate.
  • Quarter of the plate: protein. Legumes, eggs, fish, poultry, tofu. Plant and animal sources both count.
  • Quarter of the plate: intact carbs. Lentils, beans, whole grains, sweet potato. Not no-carb; better carb.
  • A glug of olive oil: nuts or seeds with meals or snacks.

This is not a meal plan. It is a way to assemble dinner without thinking too hard. Limit ultra-processed snacks and sugary drinks as a separate, parallel discipline, since the evidence on ultra-processed food intake and cardiometabolic outcomes is now consistent.

Practical patterns that help insulin

Five small habits with reproducible postprandial effects:

  1. Protein and fibre before the carbohydrate at a meal: a salad starter, or a few bites of protein, before the rice or bread. Modest but reproducible effect on postprandial glucose.
  2. Three meals a day, snacks only if hungry: constant grazing keeps insulin elevated; a meal-shaped eating pattern lets it fall between meals.
  3. A light walk after dinner: ten or fifteen minutes is enough to flatten the postprandial curve. The cheapest intervention with reliable data.
  4. Adequate sleep: sleep restriction is independently insulinogenic. Cycle tracking is exhausting; sleep is part of the protocol.
  5. Hydration: boring, but sugary drinks are one of the largest postprandial glucose contributors in most Western diets, and water replaces them.

What to do tonight

Pick one swap and do it for the next two weeks:

  • White rice to lentils or quinoa at one meal per day.
  • Sugary drink to water at one occasion per day.
  • One refined-grain snack to a piece of fruit plus a small handful of nuts.

Add fibre gradually. Going from 15 g to 35 g per day in one week is a recipe for GI discomfort. Two weeks is gentler; the gut microbiome adapts.

Plan three meals around the half-plate rule for tomorrow. That is the whole plan for the day.

When to involve your clinician or dietitian

Speak to your clinician, or ask for a dietitian referral, in any of these situations:

  • BMI under 18.5 with PCOS. The management priorities are different and lower-end body composition needs careful planning during TTC.
  • History of an eating disorder. Any new dietary protocol should be discussed first with a specialist; this is not a willpower question.
  • Type 2 diabetes, pre-diabetes, NAFLD, or other metabolic diagnosis already in your notes. Coordinate care so PCOS-fertility eating does not conflict with diabetes management.
  • Persistent anovulation despite six months of sustained dietary and lifestyle change. The next step is usually a clinical conversation about letrozole or metformin, not a stricter diet. I cover the medication side in letrozole for PCOS overview.
  • Any sense that food is taking over your thinking, or that you feel guilty after most meals. That is a clinical conversation.

The honest summary on PCOS diet supplements: the plate matters more than the bottle. Mediterranean-pattern eating, a lower glycaemic load, enough protein and fibre, and inositol where appropriate is the combination that earns its place in the guideline.
